I hereby acknowledge that I am voluntarily participating in activities organized and conducted by the Northern Lakes Arts Association (NLAA) Youth Summer Programming. In consideration for being permitted to participate in such activities, I hereby release, discharge, and hold harmless the Northern Lakes Arts Association, its directors, officers, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any loss, damage, or injury, including but not limited to bodily injury, personal injury, illness, or property damage that may be sustained by me or any property belonging to me during the course of my participation in NLAA Youth Summer Programming activities.
I understand and acknowledge that participation in NLAA Youth Summer Programming activities may involve certain risks and hazards, including but not limited to those associated with physical activities, transportation, and use of equipment. I voluntarily assume all such risks and hazards and agree to release, indemnify, and hold harmless the Northern Lakes Arts Association from any and all liability for any injury, loss, or damage, whether foreseen or unforeseen, arising out of my participation in NLAA Youth Summer Programming activities.
I further agree to comply with all rules, regulations, and instructions provided by the Northern Lakes Arts Association and its representatives during my participation in NLAA Youth Summer Programming activities.
I have carefully read and fully understand the terms of this Liability Release Statement, and I voluntarily sign it of my own free will.
I authorize the Northern Lakes Arts Association (NLAA) and its representatives to seek and consent to emergency medical treatment for my child in the event of any injury, illness, or other medical emergency that may occur during my participation in NLAA Youth Summer Programming activities. This authorization includes but is not limited to the administration of first aid, CPR, and the transfer of the participant to a medical facility for further evaluation and treatment.
I understand that every effort will be made to contact Amy child's designated emergency contact person(s) in the event of a medical emergency involving them. However, if they are unable to communicate and can not reach the designated emergency contract person(s) or if immediate medical attention is required, I authorize the Northern Lakes Arts Association and its representatives to make decisions regarding their medical treatment on my behalf.
I agree to assume full responsibility for any and all costs associated with any medical treatment provided to me during my participation in NLAA Youth Summer Programming activities, including but not limited to medical transportation, hospitalization, and physician services.
I have read and understand the terms of this Right to Treat Statement, and I voluntarily consent to the provision of emergency medical treatment as described herein.
Once programs have filled we will add you to the wait list. Upon the beginning of the camp we will issue refunds for any campers who were waitlisted.
